Food insecurity toolkit for physicians and providers

The CDPHE Food Security workgroup is piloting a toolkit with physicians and providers to share best practices and resources to address food insecurity in patients, particularly regarding enrollment of Medicaid beneficiaries in SNAP and WIC, and screening for food insecurity and connecting to nutritional resources. Food insecurity, unreliable access to a sufficient quantity of affordable, nutritious food, is a social and economic condition with widespread direct and indirect consequences. Access the toolkit here. Also consider providing feedback to CDPHE staff on the toolkit's helpfulness and utility. MAT providers - Pueblo
