Food insecurity toolkit for physicians and providers
The CDPHE Food Security workgroup is piloting a toolkit with physicians and providers to share best practices and resources to address food insecurity in patients, particularly regarding enrollment of Medicaid beneficiaries in SNAP and WIC, and screening for food insecurity and connecting to nutritional resources. Food insecurity, unreliable access to a sufficient quantity of affordable, nutritious food, is a social and economic condition with widespread direct and indirect consequences. MAT providers - Pueblo
Access a list of Medically Assisted Treatment providers in Pueblo, Colo. It has been developed with assistance from both SURE and PDPHE. Note that this list is likely to update with regularity as providers may change the type of treatment offered, such as residential vs outpatient vs detox; and others may newly join the field of treatment providers. LifeBridge wellness toolkit
In 2018, 10 county medical society executives published a toolkit to help other medical societies replicate physician wellness programs. This was published in July 2018 under the name LifeBridge and is available for download here: The Physician Wellness Website. Three in-person training dates for those exploring the startup of a Physician Wellness Program are available.
